Monitoring group issues preliminary denial to hospital, which
will plead its case in May

"Alta Bates Summit Medical Center, one of the East Bay's top
hospitals, is closer to losing its accreditation from a national
monitoring agency, the agency confirmed Wednesday.

...JCAHO found deficiencies in the following areas during its
November survey, according to a now-public report:

- Informed consent from patients
- Appropriate response to actual or potential adverse drug
events and medication errors
- Individualized and appropriate patient care and treatment
- Equal care and treatment among patients
- Pain assessment
- Following approved protocols on the use of restraints
- Complete and accurate medical records
- Appropriate actions to prevent or reduce the risk of
infections contracted during hospital stays
- Proper storage, review and management of prescriptions;

Alta Bates Summit also scored lower than most accredited
hospitals on heart-failure care.

...Of the 16,000 hospitals nationwide inspected by JCAHO, few
reach the point of being preliminarily denied accreditation.
Twelve hospitals were issued preliminary denial of accreditation
in 2004."
