The attached event report pertaining to a head injury that caused a concussion and lacerations to the top of my head contains blatant fabrications and shows either complete incompetence by the OPD or a calculated effort by the OPD to renege on its responsibility to do basic police work. I sincerely hope that this poor service is not motivated by the unfavorable publicity that I have published on the “San Francisco Bay Area Independent Media Center” (http://www.indybay.org) regarding the OPD while working as a freelance journalist.

Dear Sir or Madam:

This is a formal poor service and negligence complaint.

The attached event report pertaining to a head injury that caused a concussion and lacerations to the top of my head contains blatant fabrications and shows either complete incompetence by the OPD or a calculated effort by the OPD to renege on its responsibility to do basic police work.

I sincerely hope that this poor service is not motivated by the unfavorable publicity that I have published on the “San Francisco Bay Area Independent Media Center” (http://www.indybay.org) regarding the OPD while working as a freelance journalist.

First of all, it is outrageous to report that I told the personnel from Paramedics Plus that I injured myself by falling into an electrical box.

Paramedics Plus is, as I understand matters, largely staffed by personnel who formerly worked for the Onex Corporation's American Medical Response, Earl Riggs' Regional Medical Systems and Richard Angotti's Acme Western Ambulance.

I fully expect that the personnel from Paramedics Plus have done and will do whatever is necessary to help OPD cover up their poor service and negligence in this case.

However, there is another aspect to this case that cannot be ignored.

The event report farcically claims that that there is no evidence whatsoever aside from the information that I provided. This is an outrageous lie.

I have obtained the “Completed Incident Search” from the “Centralized Cad Journaling System” from the California Highway Patrol Golden Gate Communications Center in Vallejo.

You will find this information attached to this fax.

The 911 call made from a cellular phone that was routed to the CHP notes that the call was placed from a cell phone with a number of [number redacted to protect privacy of witness].

A simple, free reverse phone number look up on the internet shows that this cell phone is provisioned to [name redacted to protect privacy of witness].

[Name redacted to protect privacy of witness] should be contacted as part of a thorough investigation into this incident.

It should not be difficult to determine who placed the 911 call from the cell phone of [name redacted to protect privacy of witness] shortly before 6:30 PM on on November 26, 2011.

I should not be having to hold the hands of the OPD to get their personnel to do some basic police work on this incident.
